11:00 a.m. Box Lunch and Shotgun Start
4:30 p.m. Awards Reception and Silent Auction

Corporate Sponsor: Four player registrations, gift packages, box lunches, awards reception, tee sign, and recognition ($1,000)
Foursome Sponsor: Four player registrations, gift packages, box lunches, and awards reception ($600)
Individual Golfer: Individual player registration, gift package, box lunch, and awards reception ($150)
Tee Sign Sponsor: ($150) Tee sign with organziational or personal name displayed on the golf course
Awards Reception and Silent Auction: ($25 per person)
Gift: I am unable to participate, but I wish to support Phil and his quest to play on the PGA Tour with a contribution of: O $1,000 O $500 O $250 O Other Amount $________
